We are a community of public and private mental and physical health care providers working in the spirit of cooperation rather than competition to provide quality care. PCN is a not-for-profit organization that seeks to network with existing resources to provide widespread and comprehensive community health services.

About Pathways to Care

PCN is a not-for-profit network of health care providers working to decrease barriers, to educate the community, patients, and providers, and to increase collaboration to provide quality care.

Decrease barriers: PCN strives to close the funding gap, improve availability of providers and services for primary and specialty care, and find a medical home for every citizen of Josephine County.

Educate: PCN educates the community, clients, providers, and patients in all aspects of the care giving and receiving processes.

Collaboration: PCN encourages collaboration between agencies, providers, community partners, elected officials, schools, service organizations, and small and large businesses.
