PCN serves as a catalyst to care

To successfully complete projects, you need passion, people, and buy in. It is important to be proactive and enthusiastic and invite others to join in the success.

  • PCN seeks to connect patients with the quality care they need by the
most direct and cost effective path.
  • PCN is a community of public and private mental and physical health
care providers working in the spirit of cooperation rather than competition
    to provide quality care.
  • PCN is a not-for-profit organization that seeks to network existing
resources in order to provide widespread and comprehensive community
    health services.

WOW is on the road again! Walk on the Rogue, Rotary Duck Race, Flu Clinics, and at many other community events staffed by PCN members.

WOW Accomplishments in 2005

HCCSO
Health Services Strategic Plan for Josephine County approved PCN as one of its providers. The goal of the plan is to reduce infant mortality rates in Josephine County with a focus on interconceptional care. The grant is for $576,000 over 4 years. Service providers for this grant are the Siskiyou Community Health Center and Josephine County Public Health.

Wellness on Wheels
When the PCN Wellness Wagon was stolen, the community came together to solve the problem. A 2003 – 31 foot motor home was made available by Gates Home Furnishings, with the challenge to the community to make an investment in the purchase, equipment and remodel.

The Grants Pass Courier gave generous publicity and editorial support. Zonta, the Four Way Foundation and a private individual each made significant contributions, which were all matched by a contribution from Gates Home Furnishings to the purchase the motor home, and to remodel install the equipment. Logan design created the signage, the Music Shop donated the security system, Lipperts donated the vinyl, AMR will perform routine maintenance, and Lovejoy Hospice and Siskiyou Community Health Center each donated significant staff time to complete the project.

A new website has been created with the support of Rotary that will host a community calendar so scheduling can be done and also, citizens can check when the WOW will be in their community.
